In 2001, Matrix broke out of the traditional physician office visit model and now delivers innovative comprehensive health assessments—in the home—through a national network of primary care providers and nurse practitioners. Scott says they address the big-picture healthcare needs of health plan members, assessing everything from the members’ health conditions to their support systems and other environmental factors that impact their well being.

Matrix “changes lives” by providing an in-person health risk assessment service and data analytics that help Medicare Advantage plans and their members. These assessments are one-on-one, in-home health evaluations conducted by licensed health care providers that identify risk factors that should receive further medical evaluation. During visits, Matrix’s employee nurse practitioners (NPs) perform a non-invasive physical exam, ask them questions about personal, social and their family medical history, and review the member’s living environment and current medications. NPs provide health information and make recommendations for further evaluation by a primary care physician.
